Direct Deposit Enrollment Forms
Direct Deposit Benefits:

It's secure. There are no checks to be lost or stolen.

It's timely. Payments reach your account the day the check is issued -- even if you are out of town, sick or unable to get to the bank. Except for extraordinary circumstances, direct deposits are posted to your account before 9:00 AM each banking day (Monday through Friday).

It's convenient. It can save you trips to the bank and help you avoid long lines at tellers or ATMs.

You may be paid sooner. The federal government and many employers will deposit your check a day early if the regular payday falls on a holiday.

It can save you money. Direct deposit can help you avoid bouncing checks (and return item fees) because the deposit is direct and on time.
What is Direct Deposit?
Direct Deposit is the electronic transfer of a payment from a company, the government or an organization into the checking or savings account of an employee, retiree, taxpayer, or shareholder.
The most popular Direct Deposit is for payroll, but it can also be used for many other types of payments, including:
- Annuities
- Bonuses and commissions
- Dividend and interest payments
- Pensions
- Travel expense settlements
- Tax refunds
- Social Security or other government payment
How it works
By completing a standard enrollment form, you authorize a company or organization to make a Direct Deposit payment into your bank account instead of giving you a paper check. Direct Deposits are made via the Automated Clearing House (ACH) system.

How to Sign Up for Direct Deposit
Signing up for Direct Deposit is easy. You can use our online inter-active Adobe Acrobat Reader Forms:

To request Direct Deposit of your paycheck to your Pacific Trust Bank account, complete, print and sign the Payroll Direct Deposit form, and give it to your employer's human resources or payroll department (they may also want a voided check to verify bank account data).

To request direct deposit of U.S. Government payments (e.g. Social Security, SSI or military retirement pay):

Come in to any Pacific Trust Bank office, and our staff can assist you to complete and submit the required forms; or

You can call Social Security toll-free at 1-800-772-1213 (TTY 1-800-325-0778) and ask to sign up. You need a copy of your bank statement or your checkbook on hand when you call; or

You can sign up by using the U.S. Government Direct Deposit Sign Up Form. You can fill it out online, then sign it and either take it or mail it to Pacific Trust Bank or to your local Social Security office.

To start or change other types of direct deposit (such as investment income), you will need to provide the paying company or institution with the ABA number which identifies your bank and your Account Number.
